To apply for a Service, Family Service or Home Helpmate, please download and complete the application and mail to WAGS at 1338 Dewey Court, Madison, WI 53703. If you cannot download the application, please call 608-250-9247 and request an application be mailed to you. To apply for a Clinical Therapy Dog, please download and complete the Clinical Therapy Dog application and mail to WAGS.
Our staff will review your completed application upon receipt of the above and call you to discuss the information.
